Policy Agreement and Release Form

In consideration of Pawfessional Club K9, Inc.’s (hereinafter called “Pawfessional Club K9”) dog sitting services, the dog(s) owner, whose name and signature appear below (hereinafter called “Owner”), agrees to the following:

1.                  Owner certifies that he/she is the sole owner of his/her dog(s).

2.                  Owner represents and warrants to Pawfessional Club K9, that his/her dog(s) has not harmed, shown aggression or displayed any threatening behavior toward any person and/or dog. 

3.                  Owner certifies that his/her dog(s) is not a “dangerous” or “vicious” dog.

4.                  Owner represents and warrants to Pawfessional Club K9, that his/her dog(s) is in good health, has no diseases and is up to date on all of it’s shots, vaccines and other preventative treatment.

5.                  Owner understands and agrees that if his/her dog(s) become ill or injured, Pawfessional Club K9, (in its discretion) may engage the services of a veterinarian or provide medical attention to his/her dog(s).  Owner understands and agrees that he/she will have full financial responsibility for any such veterinary services or medical attention.

6.                  Owner understands that he/she is solely responsible, financially or otherwise, for any harm and/or damage caused by his/her dog(s).  Owner hereby agrees to release, defend, indemnify and otherwise hold harmless Pawfessional Club K9, and it’s directors, officers, employees, successors, heirs, executors, and all other persons associated with Pawfessional Club K9, from any and all claims arising out of his/her dog’s conduct.

7.                  Owner understands and agrees that any dog’s behavior is unpredictable and participation by his/her dog(s) in any Pawfessional Club K9, activity/service may result in injury. Pawfessional Club K9, and its staff will use reasonable efforts to minimize aggressive behavior that may result in injury.  Owner hereby assumes all risk to himself/herself and/or his/her dog(s) while at Pawfessional Club K9. 

8.                  Owner shall provide Pawfessional Club K9, with a copy of his/her dog’s most recent medical and health records by the dog’s veterinarian prior to admittance to Pawfessional Club K9, Owner will be responsible for providing Pawfessional Club K9, with updated medical and health records with in Thirty (30) days of each veterinarian visit.  All shots, vaccines and other preventative treatments described herein must be current.

9.                  Owner understands and hereby agrees that prior to admittance to Pawfessional Club K9, Owner must bring his/her dog(s) to meet for a “Get Acquainted” visit. Pawfessional Club K9, in its sole discretion, reserves the right to deny and/or refuse admittance to any dog(s) that does not meet Pawfessional Club K9’s health and behavioral requirements, or for any other reason.

10.              Owner agrees that unless other arrangements have been made in advance between Owner and Pawfessional Club K9, Owner must pay all Pawfessional Club K9’s fees and costs for services at the time of pick-up of his/her dog(s).

11.              Owner acknowledges that the following ailments/injuries/illnesses/diseases may occur to his/her dog(s) during any activity/service at Pawfessional Club K9 and hereby agrees to release, indemnify and otherwise hold harmless Pawfessional Club K9 if any such ailments should occur:

·        Bordetella (kennel cough)

·        Sore/tender/damaged pads

·        Nicks/tears/marks/puncture wounds

·        Conjunctivitis    (eye infection)

·        Exhaustion (Due to dog(s) being outside for an extended periods of time)

·        Gastrointestinal ailments

      (vomit, loose stools, diarrhea)

·        Any other ailments/injuries/illnesses/virus/bacteria/diseases not listed

11.       A veterinarian must certify the following vaccines are current prior to admittance to Pawfessional Club K9:

·        Parvo

·        Rabies

·        Bordetella

·        Distemper

·        Corona

12.       Owner agrees and is required to have the following preventative treatments preformed on his/her            dog(s) prior to admittance to Pawfessional Club K9:

·        Worm Prevention

·        Monthly Flea and Tick Preventions (must be done year around).

13.              Owner understands and agrees that this Policy Agreement & Release Form shall be in effect indefinitely.
